Agreement
1. Nature of Training and Safety Rules
I understand that firearms training involves inherent risks. During in-home classroom instruction, students are not permitted to bring or handle their personal firearms in the classroom training area. USA Veteran Firearms Training may provide training aids, inert or practice firearms, and other equipment for demonstrations and supervised dry-fire exercises. I agree to follow all instructor safety rules and directions at all times.
2. Live-Fire Range Qualification
I understand that live-fire range qualification is a required component of the applicable Illinois Concealed Carry course. For the 16-Hour Illinois CCL course, range qualification is conducted following the classroom portion of Day 1; Day 2 consists of the remaining required classroom instruction and does not include a second range qualification. For the 8-Hour Illinois CCL course and the 3-Hour Illinois CCL Renewal course, range qualification is conducted following the applicable classroom instruction. Successful range qualification alone does not constitute completion of the course. All applicable training requirements must be successfully completed before a training certificate is issued.
Live-fire activities involve firearms, ammunition, noise, recoil, hot surfaces, ejected cartridge cases, movement by other participants, and other hazards ordinarily associated with a shooting range. I understand that serious bodily injury, permanent disability, property damage, or death can occur despite the use of reasonable safety precautions.
3. Assumption of Risk
I knowingly and voluntarily assume the inherent risks associated with participating in the training, including classroom exercises, dry-fire activities, demonstrations, simulations, travel or movement within the training premises, and live-fire range activities. I understand that personal discipline, proper firearm handling, and compliance with instructor and range commands reduce risk but cannot eliminate all risk.
4. Participant Responsibilities
I agree to immediately follow all commands and instructions given by USA Veteran Firearms Training instructors and, while at a shooting range, all applicable range rules and commands. If I observe an unsafe condition, unusual hazard, damaged equipment, or other circumstance that may create an unnecessary risk, I will immediately stop participating when safe to do so and notify the instructor or appropriate range personnel.
5. Fitness to Participate
I understand that firearms training may require concentration, judgment, coordination, standing, movement, and the ability to safely handle training equipment or firearms. I agree not to participate while impaired by alcohol, illegal drugs, or any substance that prevents me from safely following instructions. I will inform the instructor of any limitation that requires a reasonable safety accommodation before participating in an activity.
6. Release and Hold Harmless
To the fullest extent permitted by applicable law, I, for myself and on behalf of my heirs, personal representatives, and assigns, voluntarily release and hold harmless USA Veteran Firearms Training, LLC, its owners, instructors, employees, agents, and representatives, and the owners or lessors of premises used for the training, from claims arising from the inherent risks of my voluntary participation in the covered training activities. This provision is intended to include claims based on ordinary negligence to the extent such a release is permitted by Illinois law. It does not waive any right or claim that cannot lawfully be waived.
7. Private Residence and Third-Party Range
I understand that classroom instruction may take place in a private residence or other premises not owned or controlled by USA Veteran Firearms Training and that live-fire qualification may take place at a separately operated shooting range. I agree to comply with reasonable safety requirements applicable to each location. I understand that a third-party range may require its own waiver or safety agreement in addition to this Agreement.
8. Course-Specific Agreement
This Agreement applies to the course identified above, including classroom instruction and the associated range qualification or training activities conducted as part of that course. Signing this Agreement does not replace any separate waiver required by a third-party shooting range.
9. Emergency Contact and Prior-Training Credit Records
If I provide an emergency contact, I authorize USA Veteran Firearms Training, LLC to retain that information for safety and emergency-response purposes as part of my training record. I understand that the emergency contact is for instructor use and is not intended to be placed on the Illinois State Police class roster.
If I request prior-training credit and upload supporting documentation, I authorize USA Veteran Firearms Training, LLC to retain that documentation with my training record for the purpose of reviewing and documenting the credit claimed. I understand that submitting documentation does not itself guarantee that credit will be accepted. I should redact Social Security numbers and unrelated sensitive information whenever possible.
10. Student Portal, ID/DL Scan, and Electronic Records
I understand that I am submitting this Agreement electronically. I authorize USA Veteran Firearms Training, LLC to use the name, email address, class information, acknowledgments, signature information, submission date, and other information I provide for class administration, confirmation of required paperwork, safety documentation, and maintenance of training/business records. I understand that my submission may be electronically associated with my student training record using identifying information such as my email address and class date.
I understand that my waiver status and, when available, a printable copy of my signed waiver record may be made available through the USAVFT Student Portal. For older or electronically stored submissions, a printable waiver copy may be reconstructed from the original electronic submission data retained by USAVFT; such a reconstructed copy is a record of the stored submission and may not be an exact visual reproduction of the screen or PDF shown at the time of signing.
If my instructor requests an ID/DL scan through the Student Portal, I understand that the PDF417 barcode on the back of my driver's license or State ID may be used to populate roster information such as my name, date of birth, and address. USAVFT does not need to retain the camera image, driver's license or State ID number, or raw barcode data for this process. I understand that authorized personnel may access retained training records only for legitimate training, safety, compliance, and business-record purposes.
11. Acknowledgment
I HAVE READ THIS RELEASE OF LIABILITY, ASSUMPTION OF RISK AND SAFETY AGREEMENT. I UNDERSTAND ITS TERMS AND UNDERSTAND THAT I AM GIVING UP CERTAIN LEGAL RIGHTS TO THE EXTENT PERMITTED BY LAW. I SIGN THIS AGREEMENT FREELY AND VOLUNTARILY.
